INFORMATION AND ACKNOWLEDGMENTS
This application for employment shall be considered open for a time not to exceed thirty (30) days and only for open positions sought to be filled by AVI-SPL Canada Ltd. at the time of the application. AVI-SPL Canada Ltd. does not accept applications for “any” or “open” positions or “general” applications.
If hired, I can provide proof that I am legally entitled to work in Canada.

I hereby certify that I understand and agree that an offer of employment with AVI-SPL Canada Ltd. is contingent upon successful completion of a reference check and a back-ground check. As well as a clean drivers abstract.
I specifically understand and acknowledge that, depending on the position applied for or the location of employment, AVI-SPL Canada Ltd. may conduct a background investigation, prior to my employment bs, concerning my character, general reputation, personal characteristics, prior criminal activity, prior driving history, or similar information, subject to and consistent with all applicable local, Provincial, and Federal laws. To this end, I authorize any company, corporation, former employer, law enforcement agency, or person to provide AVI-SPL Canada Ltd. any information that they may have regarding me, whether or not it is a matter of public record, and I specifically release any such entity from all liability for providing this information. I understand that, upon written request to AVI-SPL Canada Ltd., any investigative information obtained by AVI-SPL Canada Ltd will be provided to me within a reasonable time period. I further understand and acknowledge that I will receive appropriate notice and will be presented with a Background Investigation Authorization and Release Form from AVI-SPL Canada Ltd. in advance of any investigation AVI-SPL Canada Ltd. may perform, subject to all applicable Provincial, and Federal notice requirements.
I acknowledge that every statement made in this Application is true and correct to the best of my knowledge and belief and that submitting any false or misleading information or any omission of information pertinent to this Application is subject to discipline, including refusal to hire or termination from employment.
I acknowledge that if I am hired by AVI-SPL Canada Ltd., I will be expected to abide by any and all policies, practices, and Health and Safety procedures of AVI-SPL Canada Ltd.
